Rite Aid has partnered with SMACNA-WW to set up vaccination clinics for contractors to receive their vaccine. The JATC Office in Dupont has scheduled a vaccine clinic on Friday, May 7th. What to bring to your appointment:

Please bring your Consent Form, Billing Form, Insurance Card, and ID Card to your appointment. Also bring a photocopy of your Insurance and ID Card, front and back. The pharmacist will require all these items before administering the vaccine.
